Leviton telephone universal junction block, Ivory. Central connection for up to 4 telephone extensions support 1 or 2 lines. Modular connection to primary service. Easy way to wire up multiple phone stations.

Instructions:

Unscrew color-coded terminal screws inside junction block, raising mounting tabs to maximum height.
Run 4-conductor round phone wires from jacks to junction block.
Remove 3" of jacket from each 4-conductor wire.
Insert modular plug into telephone company demarcation jack.
Remove cover from junction block.
Insert individual color-coded wires into slots under matching color-coded terminal screws.
Mount junction block to wall.
