Surface Mount jack and cover for wall phones. Fits all standard modular wall phones. Replaces or adds a surface mount, 6-wire wall phone jack. For use with up to 2 phone lines. Tools required: flat-head screwdriver and wire cutters.

Instructions:

1. Remove existing wall jack.
2. Remove cover plate.
3. Pull line cord through to front of terminal plate.
4. Fasten jack to wall box through diagonal openings in jack.
5. Remove 2: of outer jacket from line cord.
6. Connect colored wire leads to terminals matching colors.
7. Replace cover plate.
8. Mount phone on mounting screws.
9. Replace handset on disconnected phone.
