Phone Wall Mount Plate Jack RJ12 RJ-12 3 Pack Modular White 6P6C Conductor Surface Audio Data Line Signal Hanger Bracket, Part # Leviton C2663-W, C2663W

Surface Mount jack and cover for wall phones. Fits all standard modular wall phones. Replaces or adds a surface mount, 6-wire wall phone jack. For use with up to 2 phone lines. Tools required: flathead screwdriver and wire cutters.

Instructions:

Remove existing wall jack.
Remove cover plate.
Pull line cord through to front of terminal plate.
Fasten jack to wall box through diagonal openings in jack.
Remove 2: of outer jacket from line cord.
Connect colored wire leads to terminals matching colors.
Replace cover plate.
Mount phone on mounting screws.
Replace handset on disconnected phone.
