Leviton Wall Plate Stainless Steel Telephone Jack Wall Mount Plate Modular RJ11 RJ-11 6P4C Telephone Line Cord Mount Cover with Push-On Plug Connections and Insertion Tool, Part # C0256, C-0256

Stainless steel cover with quick-connect, push-style connections and insertion tool. Fits all standard modular wall phones.

Installation:

Disconnect one phone in house by removing handset from hook before working with exposed phone wire.
Pull line cord through to front of terminal plate.
Fasten jack to wall box through diagonal openings in jack, using long screws provided, or hollow wall fasteners.
Remove 2" of outer jacket from line cord, exposing colored wire leads.
Connect colored wire leads to terminals matching by color.
Secure clear insulation film around modular jack. Replace cover plate.
Mount phone on mounting screws.
Replace handset on disconnected phone.
